Does not work with MSSQL

Installs and works fine with 6.4 or 6.5 provided you are using mysql. When requesting support from docusign, i was told they don't plan to fix the add-in for at least a year, and I should hire a programmer and write my own using their api...seriously?

Tried but Failed on 6.4 and 6.5

Brand new install of both 6.4 and 6.5 - Fails to work on both versions.
